• Please fill out the RTF form.
• Right click and select "save as" to save your document to your computer.
• You can then open the saved document (on your computer) to add additional information, or to change previously inserted information.

2.    Contact a Fellow Mentor, certified as a Fellow (CMF) to:

• Support and advise you in preparing your Application
• Confirm that the Application is complete and ready for review

3.    Determine, with your Fellow Mentor, your certification level

4.    Provide a current copy of your résumé/CV and documentation of academic degree or equivalency.

5.    Identify the competencies you can demonstrate, meeting the requirements for your Level of Certification.

• Competencies must be validated by accomplishments within the past five years to assure your experience and professional development are up to date.

6.    Select from nine Validation Methods to document competencies and calculate Professional Development Credits for your experience.

• There are three “must have” Validation Methods for all Applications:
  1. Courses/Trainings Attended and Completed
  2. Courses/Trainings/Presentations Delivered
  3. Case Study of practical experience (PDF)

7.     Insert all information required into the Application document, following specific directions for each Validation Method.

• Complete Declaration from Applicant and include in Application.

8.     Commit to the Code of Ethical Practices with your signature.

9.     Request that your Fellow Mentor review the Application and provide a letter of support.

10.     Send completed application as one e-mail attachment to ICCI and pay appropriate fees in the form of a check or online.

11.     Allow 4-6 weeks for your Application to be reviewed by the Application Review Division.

• If you are applying for CMF Certification, a Fellow Interview will be conducted. When the Application Review Division is satisfied that the Application meets the Institute’s criteria, it will recommend acceptance to the Board of Governors, who will then vote. If a majority is in favor, you will be certified and a certificate issued.
